Late Policy
Please arrive 10-15 minutes early for your appointment.
If you know you will be late for an appointment, please call us in advance to see if we can try to still fit you in. You may be asked to reschedule to another time or another day.

All new patients are required to fill out the following forms:
Transferring From Another Practice?
All patients transferring from another practice are required to fill out an Authorization For Release of Medical Records. Please download and complete the form. Most practices allow for a free transfer to another physician but some practices may charge a fee. We prefer to have all of your previous medical records before seeing a patient.
School/sports/daycare/camp/allergy/medication forms (any form that needs to be completed by our medical staff) will be completed free of charge at the time of a well visit. Please complete all of the parental sections of forms prior to presenting to our staff. Forms can be completed at other times for no additional fee with a turnaround time of approximately 2 business days. In order to have it completed at another time, the patient must be up to date on all well visits and immunizations.
Immunization records are provided free of charge at any time.
Billing
Information
All co-payments are expected at time of service, regardless of who brings the patient to the office. For your convenience, Collins Pediatrics accepts cash, personal checks, and most credit/debit cards. Surcharges will be assessed for returned checks or co-payments not made at time of service. Our contracts with insurance companies require that we verify your current coverage and collect your co-payment at each visit. Please bring your insurance card to every appointment.
Collins Pediatrics accepts most insurance plans.
If you do not see your health insurance company listed below, please contact your insurance company to see if our office is contracted with them.
Even if your insurance company is listed, certain plans under those companies are not covered, so please contact your insurance company to see if our office is contracted with them.

Insurance Cards
Please bring your current insurance card to each visit. It is your responsibility to know the benefits and provisions of your individual policy. If you do not provide us with the current insurance information at the time of service, we may not be able to file the insurance claim for you, making you, the parent or legal guardian, responsible for the cost of all services.
Prior Authorizations
Prior authorizations may be required by your insurance company for some medications, testing & labs.
Co-Payments & Billing
Please be ready to pay co-payments, deductibles, co-insurances and balances at the time of your visit. If you are unable to do so at the time of visit, we are more than willing to work with you but you must communicate this information to our billing department.
Collins Pediatrics mails statements on a monthly basis. If you receive a bill in the mail, you can call the office to pay your bill over the phone, mail in payment, or come to the office to pay it in person.

Returned Mail/ Check
Please remember to update your patient registration sheet each time you are in the office.
When a check is returned to us not paid by your bank due to insufficient funds or other reasons, the bank charges our account a fee and that cost will be added to your account balance. After 2 returned checks, we will no longer accept checks to pay your account. Your balance will need to be paid by cash or credit card.
Non-Covered Services
There are times when routine exams require testing such as labs, lead tests, and vision and hearing screening. While some insurance companies do not cover the cost of these tests, we feel that the testing is vital for a thorough exam. Please inform us if you do not wish to have these preventive services provided if your insurance does not cover the cost. If your insurance company does not cover the cost of the services, you will be responsible for the charges.
Payment Methods
Collins Pediatrics offers several options for payment: Cash, Checks, HSA cards, Visa, MasterCard, Discover or American Express.
